chore: 构建与仓库整理

- .gitignore 排除运行时产物（run/、cpa/、dist/、二进制、日志）
- dist 构建产物移出版本库（磁盘保留，发布走 Release 产物）
- Makefile/Dockerfile/docker-compose/config.yaml.example 同步 ops-assistant 更名
- 新增 VERSION 文件（0.0.1）
